Your personal history statement is an extensive document that will take some time to complete. Please allow for at least one week to complete the PHS and ensure that all fields are filled in with data or "N/A" if they are not applicable. THIS DOCDUMENT MUST BE SAVED AND FILLED OUT ON YOUR COMPUTER. DO NOT PRINT IT-This is a fillable PDF.
